Consent to Personal Data Processing Statement

In accordance with the requirements of Part 3, Article 7 of Federal Law No. 210-FZ dated July 27, 2010 "On the Organization of Provision of State and Municipal Services", Part 4, Article 9 of Federal Law No. 152-FZ dated July 27, 2006 "On Personal Data"

The personal data for which I give my consent includes:

• last name, first name, patronymic;
• passport details (series, number, date and issuing authority);
• date and place of birth;
• registration address and residential address;
• information containing home phone number, mobile phone number, personal email address.

I grant the Operator the right to request and receive documents and information necessary for the provision of state (municipal) services from state service providers, municipal service providers, organizations involved in providing state and municipal services, for the purpose of completing the document package for state services provision, as well as to transfer document packages to state (municipal) service providers.

The Operator has the right to process my personal data both with and without the use of automation tools.

I confirm that I am aware of my right to withdraw this consent by submitting an appropriate written document, which can be sent by me to the Operator. In case of withdrawal of my consent to personal data processing, the Operator has the right to continue processing personal data without my consent if there are grounds specified in paragraphs 2-11 of Part 1, Article 6, Part 2, Article 10 and Part 2, Article 11 of Federal Law No. 152-FZ dated July 27, 2006 "On Personal Data".
